The honor means Phelps will appear on the cover of the magazine's final issue of 2012, which hits newsstands Wednesday. Phelps was named Sports Illustrated's Sportsman of the Year in 2008.
Sports Illustrated readers voted for the Moment of the Year and gave the title to Phelps' performance at the 2012 London Olympics, where he won six medals, including four golds, and brought his career Olympic medal total to 22, becoming the most decorated Olympian of all time.
Phelps received 21 percent of the vote, but Gabby Douglas was not far behind; the U.S. gymnast earned 19 percent for winning individual all-around gold at the London Olympics.
Denver Broncos quarterback Peyton Manning's return to the NFL and Jamaican sprinter Usain Bolt becoming the "fastest man ever" each received 12 percent of the vote.
The San Francisco Giants' World Series win placed fifth with 8 percent of the vote.
